Congratulations to all the recipients of our Transitional Care Fellowship. The five roles have been awarded to:

This new fellowship is being hosted by Sheffield Teaching Hospitals over various sites. The encompasses a three-day programme at Hallamshire Hospital, Sheffield Childrens Hospital, and a day spent at a specialist Young Adult clinic run by the wider trust. The programme includes lectures, panel discussion, sitting on clinics and multidisciplinary team meetings.
